This deliverable is part of an on-going series of profiles from Frost & Sullivan’s Visionary Healthcare research practice, that highlight innovative models for care delivery being initiated across the globe.

Despite the individual mandate provision of the Patient Protection Affordable Care Act (PPACA) that impose penalties for failure to maintain a health plan, nearly $40 B in uncompensated care is provided annually. Essential  services needed by many homeless individuals involve care that is often inaccessible due to cost or access. Many rehabilitative programs for mental health or substance abuse have long wait times and costs $10- 15k.

Proactive screening and coaching in the community help caregivers provide varying levels of chronic disease support outside of high cost sites of care like the hospital.
